As the second Clinical Care Coordinator you will work closely with the our current CCC and Residential Care Manager to ensure the overall clinical management of consumers in line with the Aged Care Quality Standards.
This position is perfect for someone who has change management experience and has a passion to help uplift the facility they are a part of.
Full Time, Permanent vacancy which will include weekend work.
Must have AHPRA Registration and be a seasoned CCC in the aged care sector.

Role & Responsibilities: â¯â¯
- Acting as a point of contact for clinical care matters within Anglican Care’s Residential Aged Care Home.
- Individual Case Management support to the Residential Care Manager to ensure a high standard of care is provided to consumers;
- Organising and facilitating case conferencing to partner with consumers and their representatives in the development of care provisions and plans;
- Assisting to meet the goals and objectives of Anglican Care;
- Ensuring the highest standard of consumer care through audit processes;
- The development and implementation of all policies in relation to the provision of Resident Care Services;
- Providing Guidance and Leadership to other staff to ensure a high standard of care is provided to consumers
About you?
- Registered Nurse with current registration under the NSW Nurses Registration Board
- High level competence in the provision of Nursing and clinical skills within an Aged are environment.
- Demonstrated knowledge of the health and /or Aged Care sector
- Demonstrated high level skills in Aged Care Assessments, recognising and responding to acute and chronic changes, and communicating a plan of care with consumers, their families, carers and relevant employees.
- Demonstrated competence and understanding of aged care funding.
